Chimacum School District Activity Code
Washington Interscholastic Activities Association (WIAA) requires that students have 10 practices (12 for Football) before competing in a contest. As a participant in any activity, students are not eligible to practice or compete in contests until the following are completed and turned in to the office:
- Eligibility Form Eligibility Form
- Physical Examination Form (good for 24 months, all incoming 9th graders need a new physical) Physical Form
- Medical Emergency Authorization, which may include school Insurance (must purchase), if not insured. Medical Emergency Form
- Athletic Pledge Athletic/Activity Pledge
- Sport Specific Safety Form Sports Rules
- Fees Paid
a. ASB Card ($30)
b. User Fee ($50) there is a discount for students on free and reduced lunch
c. All fines previously incurred
**For fall sports, fees must be paid before the first competition.**
